We are seeking an exceptional Staff Backend Engineer / FHIR & Integrations Specialist to become the foundational technical leader for a rapidly expanding Digital Health scale-up in Germany. Your core mandate is to guide complex architectural transitions (monolith to services), own the integration domain, and act as the technical gatekeeper for quality and vendor delivery.

You will be a key builder of a European “Lighthouse” project that sets the standard for data governance, system of record, and the safe use of AI in regulated healthcare.

Your Tasks & Responsibilities:
Architect and lead the decomposition of a monolithic system into modular services using strangler-fig patterns, schema-per-domain migrations, and contract testing.
Comfortable owning and running an ADR process: writing decision records, chairing the review, saying no to a proposed extraction that isn’t ready.
Hold external vendors accountable to exit criteria, including pushing back on the vendor’s own CEO/Head of Engineering when a phase isn’t actually done.
Take full ownership of the integration domain, building seamless and scalable connections between the core platform and external German healthcare infrastructure.
Design and implement secure, DSGVO-native architectural solutions that support the platform’s transition from an internal tool to a commercially licensable B2B SaaS.
Your Qualifications:
Senior/staff-level backend engineering experience, with a strong background in Node.js/TypeScript and PostgreSQL/Prisma (experience with NestJS is a bonus).
Hands-on experience actually decomposing a monolith into services, schema-per-domain migrations, contract testing, and strangler-fig patterns.
Prior experience in a regulated or healthcare-adjacent domain, with hands-on work in healthcare interoperability standards (FHIR, HL7).
Working knowledge of authorization/RBAC design, including patient data access, consent, and audit logging.
Enough seniority and independence to operate highly autonomously.
Fluent English is a must.
Strong Pluses:
Prior experience working alongside an outsourced/nearshore dev vendor, understanding both the leverage and the friction that comes with it.
Experience standing up CI-enforced quality gates (contract tests, i18n parity checks, visual regression).
Hands-on experience with German PVS systems (e.g., Tomedo, CGM, x.comfort), KBV technical specifications, and the Telematikinfrastruktur (TI) connector landscape.
German language skills (highly beneficial for navigating KBV/TI documentation) or Spanish.
Familiarity with the latest in AI-assisted development (“vibe coding” and agentic AI).
